Thai Kratom, Mitragyna speciosa has been used traditionally in Thailand for centuries, and some Kratom use in Malaysia has been reported. In Thailand Kratom is know as ithang, kakuam, and in southern regions, thom. Thai Kratom use dates far enough back that its true beginning can't be determined. In addition to Thai Kratom being used as a narcotic drug, Thai Kratom is often used as a substitute for opium when opium is unavailable, or to moderate opium addiction. In folk medicine, Thai Kratom is often used to treat diarrhea and some Thai Kratom users use it to prolong sexual intercourse.

Nearly all Thai Kratom use is by chewing fresh Thai kratom leaves. Other ways Thai Kratom is taken include grinding up and eating fresh, dried, or reconstituted dried leaves. Some villagers use the Thai Kratom Leaves in cooking. When preparing fresh Thai kratom leaf, the vein is often extracted and sometimes salt is added to try and prevent constipation. Consumption of the Thai kratom leaf is usually followed by drinking something hot, such as warm water or coffee. Thai Kratom Leaves can also be smoked, made into a Kratom tea, or a crude resin extraction can be made. This Thai Kratom Resin Extract is made by preparing a water extract of the Thai Kratom leaves, boiling it down, and then shaping it into small balls which are rolled in a material such as flour, then stored until use. This is apparently a quite popular method of kratom consumption.

Users of Kratom in Thailand tend to be peasants, laborers, and farmers who use the kratom plant to overcome the burdens of their hard work and meager existences. Female Thai Kratom users are apparently quite rare. In some parts of the country, it was said that parents would choose to give their daughters in marriage to men who used Thai Kratom rather than men who used marijuana. The belief is that Thai Kratom users are hard working, while marijuana users are lazy. This belief is also maintained by many of the users themselves, who report beginning use because of a desire to work more efficiently, and who say using Thai Kratom gives them a strong desire to do work.

The Thai government passed the Thai Kratom Act 2486 which went into effect on August 3, 1943. This law makes planting the Kratom tree illegal and requires existing Thai Kratom trees to be cut down. This law was not found effective, since the Thai Kratom tree is indigenous to the country. Today, Thai Kratom is classed in the same enforcement group as cocaine and heroin by Thai law, and has the same penalties. One ounce of Thai Kratom extract is punishable by death. The sale, distribution and exportation of Kratom from Thailand are also banned completely, a recent report from Thailand on a local Kratom plantation being raided, trees destroyed can be found here (THAI KRATOM PLANTATION RAIDED).
